Should I use GIC for Zirconia crowns? The most important factor with crowns is the preparation and the remaining substrate. Too many times as dentists we focus on the little details and not paid enough attention to the more important factors for long term success. We always look for something or someone else to blame in life when actually we should be looking at ourselves first. If your prep is sub-gingival then you want a cement that is easy to clean up like Ketac Cem. DENTAL AMALGAM
1) Bonded (decreases cusp deflection) amalgam performs just as well as composite in class 2 restorations. Therefore the critical point to take away that tooth preparation is critical and ensuring cusps are not left unsupported.

2) The coefficient of thermal expansion of composite is greater than that of amalgam. The real world effect of expansion is negligible, in reality things like the remaining tooth left, the patient's diet and occlusion play a far more important role.

3) Secondary decay is the main cause of amalgam failure. We have actually noticed that poorly placed composites are at more risk of developing decay than amalgam in many instances.
4) Modern amalgams are safe but one can have lichenoid reactions in rare instances. There is no validated scientific data that confirm that amalgam is toxic for human use. The only increased risks are during placement and removal where mercury vapour is present (but the dosage is generally very low). We would advise all clinicians that are removing old amalgam restorations to do so under rubber dam isolation.
5) We advise all composite restorations (where realistically viable) to be placed under rubber dam. Essentially they are bonded in to place and for the best possible bond we must ensure dry and clean conditions. As well as improved patient safety we have noticed that layered restorations are cured much more thoroughly and in turn are stronger and perfume better in the long term in comparison to those not layered under rubber dam.
We must also remember that many composites contain BPA (bis phenol A - normally trace amounts in BIS GMA) - BPA free composites are available now.
